COACHING AGREEMENT
This Agreement is entered into by and between: Stride Hockey Development, LLC (coach) and (client/athlete name) and (sponsor/guardian name) whereby Stride Hockey Development, LLC agrees to provide coaching services, in exchange for services paid by Sponsor, for Client focusing on the following:
Off-ice hockey skills and development. These include, but are not limited to, skating, stick handling, shooting, speed, power, agility and hockey-IQ enhancement.
Description of Coaching: Coaching is a partnership (defined as an alliance, not a legal business partnership) between Stride Hockey Development, LLC and the Client through a periodization, or phasing, off-ice program in order to maximize personal and athletic potential. It is designed to facilitate the development of athletic (ice hockey) performance.
Coach - Client Relationship
A. Coach agrees to maintain the ethics and standards of behavior established by the International Coach Federation (ICF) (coachfederation.org/ethics).
B. Client is solely responsible for creating and implementing their own physical, mental, and athletic well-being, decisions, choices, actions, and results arising out of the coaching relationship both during and outside of all coaching sessions. The Coach will not be held liable for any results or inaction on the part of the Client.
C. Client and/ or Sponsor acknowledges any party (Client/Coach/Sponsor) may terminate or discontinue the coaching relationship at any point, but the existing financial commitment will not be refunded.
D. Client acknowledges that hockey development can be a comprehensive process that may involve different areas of focus. The Client and Sponsor agree it is the Client’s responsibility to keep the Coach informed of any relevant life changes, previous or current injuries, or any other relevant information that could impact the Coaching approach.
E. Client acknowledges coaching does not involve diagnosis or treatment of mental disorders as defined by the American Psychiatric Association and that coaching is not to be used as a substitute for counseling or any other form of mental health care. This is especially applicable to one on one virtual sessions between Coach and Client.
F. Client and Sponsor acknowledge coaching does not ensure Client performance outcomes.
G. Sponsor and Client agree all sessions must be booked via acuity:Scheduling prior to attending class. If class is canceled/rescheduled within 12 hours, you may get your credit back, but you will be charged a $15 convenience fee. If the athlete is a “no call/no show” or canceled/rescheduled within an hour of the class time, the class credit will not be credited and a $15 convenience fee will be charged.
H. Sponsor and Client understand membership packages expire after 6 months of purchase date. No refund will be granted to any unused classes.
I. Client and Sponsor agree Client must wear helmet when they are in the hockey skills side of Stride Hockey Development, LLC. Coach will not allow Client to participate in that portion of class if a helmet is not worn.
J. Coach will have a zero tolerance of disrespect (physical or verbal assault– this includes “friendly pushing”) to the facility, Coach or Client’s peers. Just simply keep your hands to yourself– it has been an issue, which is why this is in bold. The Client will get one (1) warning, depending on the degree of the offense, and the Sponsor will be notified. If there is a second offense, the Client will be removed from the program and no refund will be granted. In some extreme cases, a Client will be removed due to the first offense. Coach has the right to use sole discretion for determining the level of the offense.
K. Sponsor agrees to drop-off Client for scheduled classes, and recognizes there is NO SPACE to wait inside the facility while Client is in class and there is very limited space in the parking lot to wait, but it is permitted if the Sponsor finds a legal parking spot which also does not interfere with training.
L. Client/Sponsor agree all training items in Stride Hockey Development, LLC are property of Stride Hockey Development, LLC and will not leave the premises, this includes, but is not limited to, pucks, hockey training aids, workout equipment of any kind, etc.
M. Client/Sponsor/Coach are aware cameras with audio are operational in Stride Hockey Development, LLC and may be used for security/liability support should there be a need.
N. Coach agrees to provide at home workouts in the event the facility will be closed for 3 or more days, and such workout will not count against Client/Sponsor class credits
O. Client and sponsor agree members must attend orientation prior to attending facility
P. Client and sponsor agree that any client under the age of 14 must be accompanied by an adult when attending the facility outside of staffed hours AND “staffed hours” does not mean Stride Hockey Development, LLC is coaching or babysitting. If the client cannot behave or becomes a distraction they will be asked to leave.
Q. Client and sponsor are aware that video and audio will be in progress during staffed and unstaffed hours and will be regularly monitored and reviewed, so all equipment and persons should be respected as normal
R. Client and sponsor understand that if members are using the facility during class times, Coach(es) and classes have priority over all areas and equipment– members are expected to stay out of our way
S. All clients/members agree to pick up after themselves (return all equipment and pucks)
T. All members agree to never bring a non-member into the facility without a coach’s permission and a signed wavier. Violation of this will lead to immediate revoking of violating member and violating member will be solely responsible for any damage (to facility) or injury (of persons) to or from the non-member
*Coaching Agreement may change at any time. If any changes are made, Sponsor/Client will be asked to sign the most updated version*
Media Release Form
I hereby grant permission for video and/or images of my child, captured during Stride Hockey Development, LLC sessions to be used solely by Kathleen (Katie) Baldwin and Stride Hockey Development, LLC for promotional material and waive any rights of ownership or compensation thereto.
